To enquire about support from Rose Bay Community Care, please complete our online Service Referral Form.

The form will ask for information about the person seeking support, their care needs and the services required. This helps our team understand how we may be able to assist and prepare for an informed initial conversation.

Referrals are welcome from individuals, family members, support coordinators, healthcare professionals and other authorised representatives. Once your referral has been received, a member of our team will contact you to discuss the next steps.
